The Complex Needs team are developing a tool that they hope will help children with complex medical conditions get home more quickly – and they’d like your help in doing it!
They’re calling it the ‘Road Map Home’ and want it to show the journey that a patient goes on when they come to us at Alder Hey, giving the child and their family an idea of when they might expect to go home and what needs to happen to enable this.
The workshop is designed to give parents and carers of children with complex medical needs the opportunity to tell them:
• What good care and discharge planning should look like.
• What would make the biggest difference or be the most helpful for families being discharged from the hospital?
Patients and families are invited to an interactive session to help design the ‘Road Map’. Your knowledge and expertise in your child’s needs can help to make sure that the tool is useful for children, families and professionals. Details of the session can be found below:
