CAMHS Referral Form - Parents/Carers

CAMHS Referral Form - Parents/Carers

Referrals made via our online forms are looked at the next working day by the clinical team. If you think that the young person’s life is at immediate risk then please call 999 or go to your nearest Accident and Emergency Department.

If you feel it is not life threatening, but you need to speak to someone urgently,  then you can call our crisis care phone line 0151 293 3577, to speak to a mental health clinician 24 hours a day, 7 days a week. Alternatively you can call NHS 111 for advice or seek advice from your GP.

Alder Hey CAMHS Self-Referral Form for Parent/Carer

Alder Hey CAMHS can only accept referrals from Children/Young People and their parents/carers who have a Liverpool or Sefton GP.
If your GP's postcode is not listed above, unfortunately this means our referral service is not available to you. To find your local CAMHS team, click here.

About you as the referrer
Reason for referral
We are part of a CAMHS partnership and sometimes it is not Alder Hey CAMHS that will be the best service for the young person you care for. If this is the case we might ask one of our CAMHS partners to see the young person as their service might be better suited to their needs.
