I knew I had a right to complain

I found out how to raise a complaint from:

Help and support

I wanted / tried to get my concerns raised informally rather than make a formal complaint

I knew for certain that my care / my child’s care would not be compromised by making a complaint

I felt that I could have raised my concerns with any of the members of staff I met

I was offered support to help me make my complaint

I was able to communicate my concerns in the way that I wanted

I felt my concerns were taken seriously when I raised them

I wanted / needed someone else to raise the complaint on my behalf
